1. Click here to submit your request.
  2. Log in to your account or create a new account.
  3. Make sure your personal information is correct.
  4. If you already have a record for your dog, skip to the next step. If not, you will "Create animal record" on the top left side of the window.
  5. Once you have your dog(s) entered you will select the dog on the right hand side the dog will turn navy blue.
  6. You will then go to the lower left side of the screen and select create a submission. Note: You will do this for each dog but not each test.
  7. You will select your test using the drop down menu, note if you are testing for DM you may need to select the I didn't see the test I would like to select and go to the second drop down menu.
  8. For Cystinuria a set of questions will pop up why are you doing test, and type of sample. I am not sure if these questions pop up for the other tests.
  9. IMPORTANT in the comment box you put the discount code "MCOA2021". Along with any other information you feel may be important to the researchers to know.
  10. You will then select Create test request.
  11. In the next window, it is IMPORTANT if you are doing additional tests for the same dog to select additional tests and go through the same steps selecting the additional tests for the same dog, discount code in the comment section so you get the discount for multiple tests for the same dog.
  12. When you have ordered all the tests you want for the dog, you will select complete submission. At this point it will ask you to enter a date you are mailing your samples. You can just enter the date you think they will be mailed and just cross out that date and write the date they are actually mailed. For the health clinic the date will be May 31, 2021.
  13. PRINT 1-2 copies of this paper so that you can mail one copy with your samples and have one for your record if you want. Before you mail samples write on it the date they are being mailed.
  14. Before or after you swab your dog(s) be sure to label the outside of the swabs with your dogs information. Make sure you let them air dry away from all other contamination and if you are doing more than one dog it is a good idea to use separate counter tops or desks.

What happens next?

After you mail your samples, the time can very from 2-8 weeks depending on how busy they are. You will get an email from PennGen saying laboratory balance and a link to your account with your balance. You will go your account and pay. After you pay you will instantly get another email with your dogs results attached. You can then use this report to submit the results to OFA to enter then into the OFA database should you wish.
